I've had ATT/Yahoo from way back in PacBell days, and I've been pretty happy with them. Speeds are good, and outages are very rare.

Time Warner Cable - It's been great. I haven't had any problems.
You can get cable, phone & internet for $90 right now.
I have Fios and love it. Verizon is the only provider that works in my area.
My router fried and they brought me a new one free of charge.
Comcast was super fast in Washington. My speeds were blowing everyone elses out of the water. Got better channels than Cox, too.
We have AT&T/Yahoo DSL, and it hasn't been bad, but it isn't phenomenal either, just OK, I'm sure there are probably better and faster services out there, it just worked out with our phone that it would be the cheapest. I don't use their software, and have a router deal with the PPPoE connection, and we only had one outage when they were changing the box at the end of the street.
I have AT&T Yahoo DSL. It is ok.. I rarely test above 1200kbps, but it is $13 bucks a month. I had TW Cable (which was as fast as Ben Johnson on horse steroids), but it was $50/mo. Since I mainly look at hockey news, MMA news and Porno, AT&T gets the job done.
And the only problem I have had was due to something I did. I put in a new phone box, and my d/l speed went to ****, but uploads were fine. Couldn't figure it out so I had a tech come out. It took him like three hours, but it turns out there were these stupid filters in the box that killed my d/l. He took out the filters and it was fine!

Stay away from Charter. DSLextreme is pretty good but I have a problem with signing up contracts plus I don't have a home line since I have so many cell phones. If you can't get Verizon to give you a new modem have someone else in your family sign up for a new plan since they are considered new customers.
